How to Add and Customize the Assessment and Form Questions

In LearnWorlds, you can build, customize, and manage the content of your assessment learning activities. Whether the assessment is graded or non-graded, you can add different types of questions, structure them into sections, and enhance them with rich media widgets.


Not all question types are available across all learning activities. Each activity supports specific types of questions:


  • Exam (graded assessments) activities support only Exam-type questions.

  • Form activities support only Form-type questions.

  • Self-assessment (non-graded assessments) activities support both Exam and Form type questions.


In this article, you will see a detailed breakdown of the available question types for each activity (exams, forms, self assessments) and learn how to add, edit, and customize them. You will also learn how to enhance your questions by adding widgets


Assessment Structure

Your assessment is structured into three main parts:


1. Starting Screen: A welcome page where instructors can introduce the assessment and outline expectations. 

2. Sections with Questions: Short quizzes may include all questions in a single section, while longer assessments can be divided into multiple sections for better organization and flow.

3. Ending Screen: Shown after submission, this screen can display the score, feedback, or a message indicating that grading is pending. It supports different statuses depending on the activity type.

Types of Questions

You can choose among the following types of questions:


Exam Questions

Form Questions

If you want to delete a question or an answer choice without losing results, you can export those results before they are lost or duplicate your typeform and make changes to the new version.


Self Assessment

This is a hybrid learning activity that combines the types of questions of both Forms and Exams. It enables learners to evaluate their understanding, reflect on their progress, and even receive personalized feedback from you.


Add and Edit Questions

While creating your assessment, you can add questions manually or speed up the process using the built-in AI Assistant, which helps with writing, research, editing, and proofreading. Questions can be added manually in multiple ways:


1. From the Editor’s Top Bar: Click +Add Question.

2. Within Sections: Click +Add/Import Question under an empty section.

3. From Add/ImportNew question.

4. By hovering over the plus+ icon in an existing question.

You can move, duplicate, cut, copy, paste, hide, and delete a question by hovering over each question, clicking on it, and choosing the respective option via the side form.

To edit the question, hover over the added question and click on Edit Question. In the editing side form, you can set up each question type's settings in the tabs below:

  • General 
  • Score (if available)
  • Feedback
  • Tags

Below, you can find some universal Question Settings that are common while editing different question types:


General Tab

  • Required: This Question is mandatory. The submission cannot take place if an answer is not filled in, and the users will receive an error if the answer is not submitted.
  • Hide from Viewer: If you hide a question from the player, then it is not shown to the user and does not influence the total count of questions (and thus the total score).


Score Tab

In specific types of questions (e.g., Dropdown, True/False, Text Assignment, File Assignment, etc.) between:

  • Basic 
  • Advanced scoring


With simple scoring, you will need to give a mark for correct and wrong answers, while with advanced scoring, you can give a mark for each option, whether wrong or right.


Feedback Tab 

Similar to scoring, you can provide feedback to your users. With Advanced feedback, you can provide feedback for each question individually.


Tags Tab

You can tag users based on their particular responses. Type the tag and click enter on your keyboard.

In short text-type questions, even minor differences—such as special characters or typos—can cause an answer to be marked incorrect. For instance, if the correct answer is "LearnWorlds" but the user types "LearnWorlds." (with a dot), the system will consider it incorrect. To prevent confusion, it's important to include multiple acceptable variations of the correct answer.

Add a Widget

You can enhance your questions by adding widgets. To do this, hover over a question and click the + icon to insert a widget. Choose between the available widgets:

  • Text Blocks – Add headings and main text for instructions or explanations.
  • Buttons – Include navigation buttons or download file options.
  • Media – Enhance questions with animations, audio, images, icons, and vector graphics.
  • Embed Code – Integrate third-party content using code blocks or embedded elements.
  • Spacers – Structure the layout for better organization.
  • Math Formatting – Apply mathematical notation when needed.
  • Previous Response – Reference a user’s earlier answer for continuity. 

More specifically, you can use these widgets to create more engaging and interactive assessments:

 

Audio widget: The audio widget lets you upload an audio file (same formats as the audio learning activity). You can choose from three templates and customize the color, size, and alignment. You can also add a title and description, but these will only appear on supported templates.

Attach File widget: The attach file widget lets you upload a file that users can download by clicking a button. To avoid issues on iOS devices, make sure the "block popups" setting is disabled. You can also customize the button’s colors, text, background, and alignment. In the same attached file template, you can also import questions via:

  • Another assessment
  • Import questions via another Question bank and from an XLS file.

Math formatting: Use the Math LaTeX text widget to add mathematical expressions and equations to your assessments. LearnWorlds supports MathJax, allowing LaTeX-formatted math in various parts of an assessment for clearer, more effective content. You can find out more here.
